2024-2025 School Health Survey

The annual School Health Survey is available for school system completion through May 30, 2025. Your input is very valuable and will assist the Texas Education Agency (TEA) in understanding how your district or open-enrollment charter is implementing school health mandates and other health-related programs and policies. 

Please forward this letter and a copy of the attached survey questions to the most appropriate member of your staff to complete the survey.

Legislative Requirement

Texas Education Code (TEC), §38.0141, requires school systems to provide the agency information relating to school health and physical activity. The School Health Survey has been developed for this purpose, and the data gathered will allow the agency and policymakers to better address the health-related needs of Texas public schools and students. 

Please note, that a district designated as a district of innovation (DOI) may not be exempted from the requirement to participate in the school health survey. A state or federal requirement imposed by statute or rule applicable to an open-enrollment charter school is also applicable to a district designated as a DOI. All regulations in Chapter 38, Health and Safety of Education Code are applicable to open-enrollment charter schools. Therefore, TEC, §38.0141 is applicable to a district designated as a DOI.

Time to Complete the Survey

The survey is expected to take approximately 15-30 minutes to complete. This survey must be completed online and can be submitted only once for each school district (including a district designated as a DOI) or open-enrollment charter school. Responses should reflect the 2024-2025 academic year and may require input from different staff members within the school system to answer the survey questions completely. A copy of the survey questions is attached for your convenience.

Please coordinate with staff to ensure that there is one submission per school system completed on a single device. Should a school system submit the survey more than once, the most recent submission, even if incomplete, will be considered the final submission.
